Our practice is runned by two general practitioners, Mette Claassen and Nynke Nagtzaam, three doctor’s assistents, Kim Broekman, Wendy Breuker and Jolanda van Esch, a nurse specialists for chronic conditions, Marc de Bont, and a nurse specialist for mental health issues, Auke van den Berg.
Are you looking for a general practitioner and do you want to register? Please fill in the registration form on our website. After application you still need te come by for identification. You can also visit our practice and fill in the form at the counter. Don’t forget to bring your passport or ID-card, and your health care insurance card.
Availability and online features
It is now possible to ask for a videoconsult with the GP or nurse specialist. You can also send us an email, our fill in the contact form on our website.
In addition you can reach us by telephone. We are available from 08.00 – 17.00 hour, with exception of our lunch break (13.00 – 14.00 hour). For emergencies you can reach us also during lunch break.
To use our online services, you need to register at the patients portal. After registering we are obligated to verify your identity before we can complete your application. Accordingly, we ask you to visit our practice, and bring your passport or ID card. Hereby we want to prevent confidential medical information is linked to the wrong person.
You can also find a part of your own medical file in the patients portal. Here you can see what diagnosis the GP has registered and the advice that has been given.

Monday until Friday from 08.00 – 17.00 hour. On Tuesdays and Thursdays we offer late evening appoinments (pre-booked) from 20.00 – 21.00 hour.
Lunch break: from 13.00 – 14.00 hour.

Nynke Nagtzaam: Tuesday, Wednesday and Friday
When you are not able due to health issues to come to our practice, you can request a home visit. Home visits will be planned between 11.00 and 13.00 hour. When possible, please apply before 10.00 o’clock.
Doctor’s assistent / practice nurse
When you contact us, the doctor’s assistent will be the first one to answer your call. She can plan a consult, home visit or telephone call with your physician. She can help you with many practical issues. And you can consult her for test results.
You ca also consult her for measuring blood pressure, blood glucose, haemeoglobin, urine analysis, removal of cerumen, treatment of warts, cervical smear, removal of stitches, etc.
Nurse specialist (POH)
On Monday Auke van den Berg is present. She works as a nuse specialist for mental health issues. If you have such problems, you can discuss them with your GP, and see if the nurse specialist could be of help. Your GP can refer you to Auke. This care is fully covered by the basic health insurance.
On Tuesday and Friday Marc de Bont is present for our patients with chronic conditions like Diabetes, COPD en astma. Besides that, he can guide and give extra care to elderly patients in our practice.
On Wednesday mornings you can consult dietician Anne Baakman. You can consult her for diet and life style issues. She works for ProFitt Diëtetiek & Lifestyle Coaching.

- Hb (anaemia)
- ECG (electrocardiogram)
- Doppler (circulation blood vessels legs)
- Skin biopsy
- Cervical smear
- Culture (wound, urine, skin, throat, etc.)

We as a team recon it is important to help improve our knowledge in medical science. This is why we participate in some projects to gain more information about for example (pre)malignant skin diseases, and the use of antibiotics.
